Description

This Community of Practice provides a space for workforce team members working with populations affected by HIV in the rural areas of Delaware to interact regularly, network, address local challenges, discuss successes, and develop best practices for linkage and care coordination.

Objective:

Understand the unique challenges faced by rural healthcare teams.
Set goals for improving care, sharing best practices and providing support for staff.
Develop best practice and educational tools to be posted in a shared online space.
