VCU received request for TA from Janet Edmonston, Grants Manager at Carilion Clinic. She asked for TA regarding documenting NMCM progress notes based on feedback she received from Peer Review. Dir. of Ed, Rob Rodney, MSW and Health Educator, Kasey Ingram, MSW met with Janet via zoom to provide TA.
Currently, the team documents all progress notes on a spreadsheet containing all clients and all encounters, rather than in the individual client charts. Reviewed HIPAA guidelines and CM standards for documentation and progress notes. Made the recommendation the Carilion starts charting NMCM progress notes at the client level in either paper charts (currently the process for MCM clients) or in EPIC EMR.
