In this regularly scheduled meeting of representatives from YesCare, Action Wellness, Philadelphia
FIGHT, AACO, and city leadership, DD’Alessandro facilitated a discussion about HIV intake testing, the
referral process for inmates testing positive for HIV, and other populations (individuals with HCV or receiving MAT while incarcerated) with discharge/continuity of care issues. There were 13 stakeholders
in attendance. Kimberly Sommers, MD attended her first Discharge Planning meeting today. She is an
Infectious Disease physician with YesCare who works full-time with Dr. D. There were issues with
getting referrals for about a month due to reporting problems – now ECW is back online and Taylor
noted that they are receiving reports/referrals consistently. The process for restarting IPs’ medical
insurance was discussed.
