This is a virtual class. The class will be offered on Zoom. As such all students must have access to a working webcam and mic to ensure the success of the training. Students without a working webcam and mic will be dismissed from the class and encouraged to register for another cohort.
Prerequisite: Successful completion of Foundations of Case Management is required for entry into this course.
This course will be run in-person. This is not a virtual class.
Objectives:
By the end of this course, participants will be able to:
- Relate a general definition of HIV/AIDS Medical & Non-Medical CM
- Describe the shift from general case management to a medical/clinical case management model.
- Demonstrate Screening, Intake, and Assessment procedures.
- Develop an Individual Service Plan.
- Demonstrate knowledge of appropriate indications for reassessment and discharge.
- Identify resources for QA Technical assistance.

Certificates will be provided upon completion of this program which confirm 6.5 hours of attendance.
