Prerequisite: Successful completion of Foundations of Case Management is required for entry into this course.
This course will be run across 2 sessions. Attendance at all two sessions is required. Web cam access is also required to participate. The sessions will run:
- Monday, May 24 9am - 12:30pm
- Tuesday, May 25 9am - 12:30pm
Objectives:
By the end of this course, participants will be able to:
- Relate a general definition of HIV/AIDS Medical & Non-Medical CM
- Describe the shift from general case management to a medical/clinical case management model.
- Demonstrate Screening, Intake, and Assessment procedures.
- Develop an Individual Service Plan.
- Demonstrate knowledge of appropriate indications for reassessment and discharge.
- Identify resources for QA Technical assistance.
Certificate of Completion
Certificates will be provided upon completion of this program which confirm 6.5 hours of attendance.
