Health Educator, Rob Rodney, received a request for assistance from Jennifer Wood, MCM of Harrisonburg Community Health Center. Jennifer asked for clarification on which version of the MCM referral form should be used during intake. Jennifer noted two different versions.

Rob researched with contacts at VDH as there have been revisions to the standards since their release in 2015. In researching VDH's website, Rob noted that full standards has 12 question version of MCM referral form while NMCM standards has 13 question version. Rob also noted that updates to ISP approved in 2018 were not reflected in either version on VDH website. (ISP now has a spot for client to initial that they've received a copy as required in CM Standard 4.2, page 15.)

Rob brought these findings to the attention of VDH DDP Quality team, Safere Diawara & Jonathan Albright. Team confirmed 13 questions and updated ISP should be used and will be updating VDH website.

Rob informed Jennifer and sent correct versions of ISP and MCM referral.
